  • I will review the LTD denial letter, highlight dates of service, providers and health issues mentioned in the denial letter.

  • Organize medical records by dates of service and/or providers.

  • Prepare separate list of other providers and/or dates of service mentioned in the denial letter that are not included in the medical records you provide for me to summarize.

  • Highlight strengths and weaknesses or items the attorney needs to look at.

  • Highlight in the medical summary the dates of service mentioned in the denial letter.

  • Prepare medication chart, include definition of medication and what prescribed for.

  • Look up healthcare provider names on health related boards licensure website to confirm board certification and specialty.

  • At the end of the summary include definitions of medical diagnoses, conditions, procedures or diagnostic tests.

Demand Letters for Personal Injury

My desire is to prepare demand letters for personal injury cases using the format that your office is accustomed to using or we can use one of my formats.

Demand letters can include the following sections:

  • Opening paragraph addressed to the insurance agency.

  • List of attached enclosures or supporting documentation.

  • Facts of the collision and liability.

  • Property damage, including photos.

  • Medical treatment, list of injuries.

  • Medical treatment summary, summarized by provider or by date of service.

  • List medical expenses/damages.

  • List other damages, such as mileage or loss of work.

  • Closing paragraphs, duties, loss of enjoyment of life, case law.

  • Settlement proposal.

Comparison Charts: Medical Bills and Insurance Claims Reports

At times the insurance claims report from the client’s health insurance company does not quite match up with the itemized statements obtained from the providers.

For subrogation purposes it is important that these 2 items match. And for your list of damages sometimes there are ancillary charges that are revealed in the claims report.

I can prepare a comparison chart of these 2 lists by date of service, identify charges on the health insurance claims summary that are not related to the accident and/or identify charges related to the accident that have not been captured by the claims summary.

The chart can show date of service, provider name, description of treatment or type of visit, amount the provider billed insurance, amount paid by insurance, amount paid by client and adjustments.

Medical Expert Notebooks

Experts and treating physicians are thrilled to have all of the pertinent medical records organized in a notebook with a table of contents and the medical summary at the front of the notebook in order to prepare for a deposition or testimony.
